Welcome to Jackson Creek Memory Care, located in Independence, Missouri, where we specialize in compassionate Alzheimer’s / Dementia and long-term care for Jackson County residents and beyond.

What to expect:

Upon move-in, usually within the first week if possible, we have an interdisciplinary team meeting with you as a family to learn just as much as we can about your loved one! Nursing, Dietary, Activities, and Social Services will meet with you and ask lots of specific questions to gain as much information as we can about him/her. After that, every quarter, we have care plan meetings with you. At that time, there shouldn’t be much new information to learn about, but we want to ensure that we are all on the same page together, all as part of your loved one’s care team – update on medications, weight, how active he/she is and whatever else is pertinent at that time.

With our activity department, we host quite a few events throughout the year in which families are invited and encouraged to attend. You’ll see notices throughout the building about these and we will attempt to reach you individually closer to the dates. These would include Thanksgiving, Christmas, Mother’s Day, Father’s Day, and other celebrations in between!

Our administration team all have open doors and welcome families to come to us with any questions or concerns. We all want to know you and for you to feel comfortable with your loved one in our care. Please let us know if there is anything we can do, to make you feel more comfortable. Also, never hesitate to let us know if we have staff going above and beyond as well! We LOVE hearing these things and celebrating them!
